Position Overview

We are actively seeking a dedicated and professional Nurse Practitioner to join our comprehensive healthcare team at Atlantis Athletes Physical Therapy Care in Islip, New York. This full-time position is focused on providing exceptional primary care to our patients, primarily older adults on Medicare, within a home care setting. Work Environment

The Nurse Practitioner will work onsite in a home care environment, collaborating as part of a multidisciplinary care team that includes physicians, physician assistants, medical assistants, and dedicated support staff such as registered nurses and care managers. Key Responsibilities

• Deliver exceptional primary care services by conducting thorough assessments and formulating personalized care plans for patients.
• Perform routine and acute office visits, utilizing evidence-based protocols to diagnose and manage health conditions.
• Conduct annual wellness visits and comprehensive health risk assessments, requiring a holistic perspective and meticulous documentation.
• Coordinate care effectively with other providers, specialists, and testing facilities to ensure patients receive timely and appropriate interventions.
• Lead initiatives in population health, such as overseeing preventive screenings and managing patient follow-ups.
• Assist in phone triage, conducting outreach to ensure patients engage with their care plans and respond to any changes in their health status.
• Educate patients and their families about medical conditions, treatment options, and self-management strategies to empower them in their health journeys.
• Engage in promotional activities to support the growth of the practice and foster community relationships.
• Conduct home visits as required, ensuring that care extends beyond the office setting.
• Collaborate closely with the Medical Director and operational leadership to achieve organizational objectives and enhance patient outcomes.

Patient Population / Client Focus

This role primarily involves providing care to older adults on Medicare, many of whom may have complex medical and social needs. The objective is to foster an environment that encourages patients to lead active and fulfilling lives while managing their health effectively. Understanding the unique needs of this population is critical, as you will be responsible for tailoring care strategies that focus on both immediate health concerns and long-term wellness.

Qualifications

• A Master of Science in Nursing, Doctor of Nursing Practice, or successful completion of an accredited Nurse Practitioner (NP) program is required.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC is mandatory.
• Possession of an active, non-probationary state Nurse Practitioner license and an active DEA license.
• Demonstrated experience in primary care settings, particularly in geriatrics, internal medicine, or family medicine.
• Familiarity with HCC documentation, ICD-10 coding, and conducting health risk assessments is essential.
• A strong passion for collaboration and teamwork across various healthcare disciplines is crucial for success.

Preferred Experience

• Experience in an outpatient or home care setting is highly desirable.
• Knowledge and experience in patient education and community health initiatives are advantageous.

Compensation and Benefits

This position offers a competitive salary ranging from USD 120,000 to 160,000 annually, commensurate with experience and qualifications.
