Work Environment

This position is based onsite at a dedicated school facility in Babylon, New York, where you will work closely with an interdisciplinary team that includes physicians, physician assistants, medical assistants, and registered nurses. Key Responsibilities

• Provide exceptional primary care tailored to the unique needs of each patient, ensuring high standards of health outcomes.
• Conduct office visits to address routine, urgent, and chronic health issues, establishing a trusting relationship with patients.
• Administer annual wellness screenings and health risk assessments, ensuring meticulous documentation and follow-ups.
• Coordinate care with other healthcare providers and specialists, facilitating comprehensive treatment plans and referrals.
• Lead initiatives in population health management, striving to ensure that all eligible patients receive essential screenings, such as biannual breast cancer screenings for women.
• Engage in phone triage and community outreach to optimize patient access to care and resources.
• Educate patients on their health conditions, presenting clear and actionable care plans to promote collaboration in their treatment.
• Participate in community health promotions and educational activities aimed at elevating health awareness.
• Perform home visits when necessary to ensure continuity of care for patients with mobility challenges.
• Report directly to the Medical Director while collaborating closely with operational leadership to enhance the quality of services offered.

Patient Population / Client Focus

Our primary focus is providing personalized care to older adults enrolled in Medicare. We aim to equip our patients with the knowledge and support they need to maintain their health and wellness. By developing strong, trusting relationships, our Nurse Practitioners empower patients to engage actively in their healthcare journey, enhancing adherence to treatment plans and improving overall health outcomes.

Qualifications

• Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or completion of an accredited NP Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner from AANP or ANCC.
• Current and unrestricted state Nurse Practitioner license with active DEA registration.
• Proven experience in primary care settings such as internal medicine, geriatrics, or family medicine.
• Strong understanding of HCC documentation, ICD-10 coding, and Health Risk Assessments (HRAs).
• A deep commitment to teamwork and a collaborative spirit to work effectively across various healthcare roles.

Preferred Experience

While not mandatory, candidates with a background in community health initiatives or experience working with geriatric populations will be highly regarded. Familiarity with innovative care models and an aptitude for employing technology in healthcare delivery will be advantageous.
