Work Environment

This position is based onsite, with healthcare services provided directly in patients’ homes throughout Babylon, New York. You will collaborate closely with a network of healthcare professionals to ensure seamless care delivery. Key Responsibilities

• Conduct thorough initial patient assessments, including detailed history and physical examinations to formulate personalized care plans.
• Maintain and update accurate patient records, ensuring all documentation reflects the patient’s current condition and treatment plan.
• Analyze diagnostic tests and collaborate with patients to determine follow-up appointments and treatment options based on clinical findings.
• Deliver high-quality care while adhering to the organization's productivity metrics and best practices for patient management.
• Act as a primary resource for patients, offering counseling and guidance to those with chronic conditions or complex healthcare needs.

Patient Population / Client Focus

The Nurse Practitioner will work with a diverse population of patients ranging from pediatric to geriatric, many of whom have chronic illnesses or require rehabilitation services. Your role will involve developing rapport with patients and their families, helping them navigate their health challenges while fostering an environment of trust and empathy.

Qualifications

• Active Nurse Practitioner license in New York State.
• Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited institution.
• Board certification as either a Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP), or a comparable certification.
• Demonstrated proficiency in chronic disease management and preventative care strategies.
• Familiarity with electronic health record (EHR) systems, particularly with respect to medical documentation and patient management.

Preferred Experience

While a minimum of 2 years of clinical experience in an outpatient or primary care setting is ideal, new graduates who have completed robust clinical rotations are highly encouraged to apply. Strong clinical skills, a patient-centered approach, and the ability to effectively manage multiple cases are essential.
