Work Environment

This role is primarily an onsite position, focusing on home care delivery. Key Responsibilities

• Conduct comprehensive evaluations and assessments of each patient to understand their physical therapy needs as directed by their physician.
• Develop, implement, and modify individualized treatment plans tailored to the specific diagnoses and recovery goals of patients.
• Provide skilled therapeutic interventions in the patient's home, utilizing techniques and modalities that align with the established plan of care.
• Educate patients and their caregivers about exercises, recovery processes, and self-management strategies to enhance overall treatment success.
• Maintain accurate documentation of patient progress and treatment results to ensure effective communication with the healthcare team and compliance with regulatory standards.
• Demonstrate exceptional customer service, ensuring a supportive and positive experience for every patient.
• Collaborate with an interdisciplinary team to maximize therapy outcomes while actively participating in team meetings and case discussions.

Patient Population / Client Focus

In this role, you will manage a diverse caseload that may encompass a variety of specialties including orthopedic injuries, pediatric conditions, neurological disorders, and sports medicine cases. Qualifications

• Bachelor's degree in Physical Therapy from an accredited program recognized by the American Physical Therapy Association (APTA) or Commission on Accreditation in Physical Therapy Education (CAPTE).

• Basic Life Support Certification accredited by the American Heart Association.
• Strong communication skills, both written and verbal, allowing for effective patient interaction and teamwork.

Preferred Experience

While not mandatory, previous experience in home health care settings or a solid background in modalities relevant to physical therapy can greatly enhance your application.
