Position Overview

Advanced Cardiology PC is seeking dedicated and compassionate Nurse Practitioners to join our team in Babylon, New York. In this full-time role, you will contribute to the delivery of high-quality primary care services to Medicare adults, with an emphasis on keeping patients healthy through collaborative and personalized care practices. The expected salary for this position ranges from $120,000 to $160,000 annually, commensurate with experience.

Work Environment

This is an onsite position at our hospital facility in Babylon, New York. As part of a collaborative care team, you will work alongside physicians, nurse practitioners, physician assistants, medical assistants, and registered nurses, supported further by care managers and a dedicated medical scribe. Our facility is equipped with state-of-the-art resources that empower our clinical team to focus on patient care, care coordination, and population health management.

Key Responsibilities

• Provide exceptional primary care, including conducting comprehensive office visits for both routine and acute health issues.
• Administer Annual Wellness Visits and Health Risk Assessments, requiring thorough documentation that reflects a holistic approach to each patient’s health profile.
• Coordinate care with other providers and specialists, ensuring seamless transitions and follow-ups for all patients.
• Lead initiatives aimed at improving population health, such as ensuring all eligible female patients receive timely and evidence-based breast cancer screenings every two years.
• Support the care team by handling phone triage and conducting patient outreach as necessary.
• Educate patients and their families about medical conditions, treatment plans, and preventive care strategies to enhance understanding and compliance.
• Engage in promotional activities to bolster community health education and awareness.
• Conduct home visits for patients who require in-home assessments or care, when appropriate.
• Perform other clinical duties as assigned, including administrative support to the Medical Director and operational leadership.

Patient Population / Client Focus

The primary focus of your role will be the care of adult patients, especially those aged 65 and older, on Medicare. Our innovative care model emphasizes preventive health, enabling older adults to live life to the fullest while avoiding unnecessary hospital stays. You will play a crucial role in ensuring that these patients receive tailored care that respects their unique needs and circumstances, ultimately enhancing their quality of life.

Qualifications

• A Bachelor’s degree in Nursing, with a Master of Science in Nursing, Doctor of Nursing Practice, or completed Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through the AANP or ANCC.
• Possession of an active, non-probationary state Nurse Practitioner license.
• Valid DEA license to prescribe medications.
• Experience in primary care settings with a focus on internal medicine, geriatrics, or family medicine.
• Familiarity with HCC documentation, ICD-10 coding, and Health Risk Assessments is essential.

Preferred Experience

• Prior experience in a collaborative healthcare environment is highly desirable.
• Hands-on experience with chronic disease management, preventive care, and community health initiatives.
• Strong interpersonal and communication skills, with a passion for patient-centered care and teamwork.
