Work Environment

The position is based onsite at our Assisted Living Facility in Queens Village, NY, a vibrant community that requires compassionate healthcare services. Our facility fosters a supportive, collaborative environment where healthcare professionals work together to ensure the best possible outcomes for our residents and patients. The expected work schedule spans from 9:00 a.m. to 6:00 p.m., with occasional evening clinic hours until 8:00 p.m. Participation in community outreach events during weekends may also be required, allowing you to engage directly with the community we serve.

Key Responsibilities

• Conduct comprehensive health assessments and screenings tailored to the unique needs of individuals and families residing within the facility.
• Diagnose and manage various medical conditions across the spectrum, utilizing evidence-based practices to develop appropriate care plans.
• Provide preventive care consultations focused on health education, lifestyle modifications, and wellness promotion.
• Collaborate closely with community organizations to design and implement health education programs aimed at increasing health literacy.
• Advocate for patients’ rights, ensuring access to necessary healthcare services and resources.
• Coordinate an efficient referral process and follow-up care, ensuring continuity for all patients.
• Maintain detailed, accurate medical records in compliance with established regulatory standards.
• Actively participate in community outreach initiatives such as health fairs to offer screenings and disseminate vital health information.
• Stay informed about community health trends, public health policies, and advancements in medical practice to enhance care delivery.

Patient Population / Client Focus

In this role, you will serve a diverse population within the assisted living facility, primarily focusing on older adults with complex health needs. Your approach will involve cultural sensitivity and an understanding of socio-economic factors that influence health outcomes. You will be instrumental in bridging the gap between traditional healthcare services and the unique needs of the community.

Qualifications

• Current certification as a Family Nurse Practitioner is mandatory.
• An active nursing license in the state of New York must be maintained.
• A Master’s degree in nursing from an accredited program is required.
• A minimum of 3 years of clinical experience, particularly in community health or public health settings, is essential.
• Exceptional interpersonal communication skills are necessary, particularly when engaging with diverse patient populations.

Preferred Experience

• Experience in geriatric care or working in assisted living environments is highly desirable.

Compensation and Benefits

The compensation for this full-time role ranges between USD 180,000 and USD 200,000 annually, commensurate with experience and qualifications.
