Work Environment

As a Nurse Practitioner in our practice, you will work onsite in a dynamic and supportive environment that prioritizes patient-centered care. With an average workweek of approximately 40 hours, your schedule will typically span from 9:00 a.m. to 6:00 p.m., with some evening clinics available until 8:00 p.m. Additionally, you may be expected to participate in community outreach events and health fairs during weekends, embodying our commitment to community service.

Key Responsibilities

• Conduct thorough health assessments, screenings, and evaluations for patients of all ages, ensuring a comprehensive understanding of their healthcare needs.
• Diagnose and manage a range of acute and chronic medical conditions, developing individualized treatment plans that encourage preventive care and promote long-term health.
• Collaborate effectively with community organizations and stakeholders, creating and implementing educational health programs aimed at empowering individuals with knowledge about health maintenance.
• Deliver culturally competent care while advocating for patient rights and access to healthcare services, ensuring inclusivity in health interventions.
• Coordinate follow-up care and referrals to specialists, guaranteeing seamless continuity of care for patients transitioning between different levels of the healthcare system.
• Maintain accurate and detailed medical records, adhering strictly to regulatory standards of documentation to support quality care and compliance.

Patient Population / Client Focus

In this role, you will provide care primarily to individuals and families within the Hempstead community who may face barriers to accessing traditional healthcare services. Qualifications

• Current certification as a Nurse Practitioner.
• Active nursing license in the state of New York.
• A Bachelor’s degree in nursing, with a Master’s degree in nursing preferred.

Compensation and Benefits

We offer a competitive salary range of $120,000 to $220,000 annually, commensurate with your experience and qualifications.
