Full-Time Nurse Practitioner for Concierge Medicine

Position Overview

United Medical Clinic is seeking a dedicated full-time Nurse Practitioner (NP) to join our dynamic team in New Hyde Park, New York. This role focuses on providing high-quality primary care in a concierge medicine setting, emphasizing personalized patient interactions and holistic health outcomes. Work Environment

Operating primarily on-site, our clinic fosters a supportive team atmosphere. As a Nurse Practitioner, you will be an integral part of a collaborative care team, including physicians, physician assistants, medical assistants, care managers, and dedicated medical scribes. Together, our team works synergistically to ensure streamlined operations and coordinated patient care, thereby enabling our healthcare providers to focus on their patients without being burdened by administrative tasks.

Key Responsibilities

• Deliver exceptional primary care services, including conducting comprehensive office visits for routine evaluations and acute health concerns.
• Administer annual wellness visits and health risk assessments, emphasizing thorough documentation and personalized health considerations.
• Coordinate care seamlessly with other healthcare providers, specialists, testing facilities, and relevant agencies to ensure integrated health outcomes.
• Take the lead in population health initiatives, ensuring that all eligible patients participate in preventive screenings and health promotion activities.
• Assist with phone triage and outreach, providing timely responses to patients’ needs and inquiries.
• Educate patients and their families on health conditions, treatment plans, and preventive care strategies, reinforcing their engagement in their own health improvement.
• Participate in promotional activities that support the clinic’s mission and increase community awareness of our services.
• Conduct home visits when necessary, ensuring continuity of care for patients who may be homebound.
• Complete other tasks as assigned, while reporting directly to the Medical Director and collaborating with operational leaders.

Patient Population / Client Focus

Qualifications

• Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or completion of an accredited Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through the American Association of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
• Possession of an active and unrestricted Nurse Practitioner license in New York state.
• DEA license must be current and non-probationary.

Preferred Experience

• Previous experience in primary care settings, particularly in internal medicine, geriatrics, or family medicine.
• Strong understanding of HCC (Hierarchical Condition Categories) documentation, ICD-10 (International Classification of Diseases, 10th Revision) coding, and Health Risk Assessments (HRAs).
• Ability to work collaboratively in a team-oriented environment with a focus on empirical-guided population health strategies.
• Bilingual proficiency in any relevant languages is a plus.

Compensation and Benefits

The salary for this position ranges from $180,000 to $200,000 annually, commensurate with experience and expertise. In addition to a competitive salary, we offer a comprehensive benefits package that includes:
