Full-Time Nurse Practitioner in Home Care - Cardiology & Internal Medicine

Work Environment

This position is primarily onsite, requiring direct patient interaction in a home care setting. Working hours are structured to accommodate our patients, generally from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m. Our team operates in a collaborative environment, ensuring that all practitioners are engaged in a culture of continuous learning and professional growth.

Key Responsibilities

• Conduct thorough health assessments and screenings for individuals and families, focusing on cardiology and internal medicine needs.
• Perform diagnostic procedures such as stress tests and order essential diagnostic imaging including chest X-rays to aid in patient evaluation.
• Treat patients diagnosed with Peripheral Arterial Disease (PAD) and manage their comorbidities to improve overall health outcomes.
• Develop and implement tailored treatment plans for patients with hypertension, diabetes, and high cholesterol, incorporating medication management for conditions such as congestive heart failure (CHF) and atherosclerosis.
• Diagnose and manage both acute and chronic medical conditions effectively while promoting preventive care and health education strategies.
• Collaborate with local organizations and healthcare stakeholders to design and execute community health education programs, enhancing public awareness and preventive health measures.
• Advocate for patients' rights, ensuring they have access to quality healthcare services and culturally competent care.
• Coordinate referrals and facilitate follow-up care to guarantee seamless continuity of care for patients.
• Maintain accurate and detailed medical records in compliance with healthcare regulatory requirements and organizational protocols.
• Engage in community outreach initiatives and health fairs to promote health awareness and provide essential health screenings.
• Stay informed about emerging health trends, evidence-based practices, and relevant healthcare policies that could impact patient care.

Patient Population / Client Focus

In this role, you will serve a diverse patient population that includes adults and elderly individuals managing chronic conditions typical of cardiology and internal medicine. You will have the chance to create meaningful relationships with patients, addressing their unique health concerns and empowering them through education and support.

Qualifications

• A Bachelor’s Degree in Nursing; an advanced degree is a plus.
• Current Nurse Practitioner licensing in the state of New York.
• A minimum of one year of prior experience as a Nurse Practitioner, preferably in a cardiology or internal medicine setting.
• Strong clinical assessment, diagnostic, and intervention skills.
• Excellent communication and interpersonal skills.
• Ability to work collaboratively in a team-oriented environment.

Preferred Experience

• Experience working in a home care or outpatient setting.
• Knowledge and experience with EHR systems for maintaining patient records.
• Familiarity with community health resources and organizations.

Compensation and Benefits
