Work Environment

The Nurse Practitioner will work onsite in a dynamic outpatient office setting, engaging closely with physicians and healthcare staff in a collaborative environment. The typical work schedule will be from 9:00 a.m. to 6:00 p.m., with the expectation of occasional evening clinics until 8:00 p.m., allowing flexibility and adaptability to meet patient needs. With an average workweek totaling approximately 40 hours, our NP will enjoy a balanced workload that prioritizes both patient care and practitioner wellness.

Key Responsibilities

• Conduct thorough health assessments and screenings for individuals and families, taking into account their unique health profiles and concerns.
• Administer stress tests, interpret results, and recommend appropriate treatment plans based on findings.
• Order and evaluate chest X-rays and other diagnostic tests as necessary to diagnose conditions accurately.
• Manage patients suffering from Peripheral Arterial Disease (PAD) and other cardiovascular issues, ensuring tailored care strategies.
• Oversee the management of chronic conditions, including hypertension, diabetes, and high cholesterol, adjusting treatment plans according to individual patient responses.
• Implement medication management for patients with Congestive Heart Failure (CHF) and atherosclerosis, ensuring adherence to best practices in pharmacological care.
• Diagnose and manage both acute and chronic medical conditions, emphasizing preventive care and health promotion initiatives.
• Collaborate with local organizations and stakeholders to create and implement effective health education programs that address community needs.
• Advocate for patients’ rights and ensure equitable access to healthcare services, providing culturally competent care to enhance patient relationships.
• Coordinate referrals and ensure follow-up care, maintaining continuity for patients transitioning through different levels of care.
• Maintain accurate and detailed medical records adhering to regulatory standards, utilizing electronic health record (EHR) systems to ensure high-quality documentation.
• Participate in community outreach initiatives and health fairs, providing health screenings and raising awareness about cardiovascular health.
• Stay informed of current community health trends, incorporating evidence-based practices in all aspects of care.

Patient Population / Client Focus

The patient population we serve primarily includes adults experiencing cardiovascular conditions and internal medicine needs, ranging from preventative care to complex chronic illnesses. Qualifications

• A Bachelor's Degree in Nursing (Bachelor of Science in Nursing or equivalent) is required.
• A valid Nurse Practitioner license in New York State is mandatory.
• Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications are preferred.

Preferred Experience

While we are willing to train and welcome new graduates, a minimum of one year of relevant experience in a clinical setting is preferred, particularly in cardiology or primary care. Familiarity with electronic health record (EHR) systems is advantageous.
