Key Responsibilities

• Conduct thorough health assessments and screenings tailored to the needs of individuals and families in the community.
• Diagnose and manage a variety of acute and chronic medical conditions, providing preventive care, education, and health promotion strategies.
• Initiate and lead collaboration with local organizations and stakeholders to develop and implement educational health programs designed to meet community needs.
• Advocate for patients, ensuring culturally sensitive care while promoting their rights and access to healthcare services.
• Coordinate patient referrals and ensure follow-up care to maintain continuity in treatment and support overall health.

Qualifications

• Current certification as a Nurse Practitioner and an active nursing license in the state of New York.

Compensation and Benefits

We offer a competitive salary range of $120,000 to $220,000 annually, commensurate with your experience and qualifications.
