Work Environment

The Nurse Practitioner will work onsite at our hospital facility, primarily interacting with patients and families in a supportive setting that promotes teamwork and mutual respect. Our typical work schedule begins at 9:00 a.m. and concludes at 6:00 p.m. with occasional evening clinics until 8:00 p.m. Additionally, participation in community outreach events or health fairs during weekends may be required, enhancing your connection with the community you serve.

Key Responsibilities

• Conduct thorough health assessments and comprehensive screenings to identify health needs across a diverse patient population.
• Diagnose and manage both acute and chronic medical conditions, ensuring the delivery of preventive care and health promotion strategies tailored to individual needs.
• Collaborate closely with local community organizations and stakeholders to design and launch effective health education programs aimed at raising community awareness.
• Provide culturally competent care by recognizing and addressing the unique backgrounds, languages, and beliefs of the patients.
• Ensure continuity of care by coordinating referrals and follow-up services, facilitating smooth transitions between different levels of care.
• Maintain accurate and detailed medical records using our electronic health record (EHR) systems in compliance with all regulatory standards.
• Engage actively in community outreach initiatives, including health fairs, to educate the public and provide essential health screenings.
• Stay updated on current community health trends, evidence-based practices, and legislative changes that impact healthcare delivery.

Patient Population / Client Focus

This role primarily focuses on underserved and at-risk populations within Jersey City. You will work closely with a diverse array of patients, including families from varying socioeconomic backgrounds and those experiencing barriers to healthcare access. By addressing the unique needs of these communities, you will play a critical role in reducing health disparities and improving overall health outcomes.

Qualifications

• A current Nurse Practitioner certification and active state nursing license are mandatory.
• A Bachelor’s degree in nursing is required; a Master’s degree from an accredited nursing program is preferred.
• A minimum of three years of clinical experience, ideally in community health or public health settings, is essential.
• Excellent interpersonal and communication skills are crucial, especially for effective interaction with diverse populations.
• A demonstrated commitment to social justice and health equity is a must, along with advocacy for vulnerable communities.

Preferred Experience

Compensation and Benefits

• A competitive annual salary between USD 150,000 and 180,000 , commensurate with qualifications and experience.
