Lead Nurse Practitioner for Innovative Primary Care in Assisted Living

Position Overview

Next Horizon Health Care is seeking a dedicated and experienced Nurse Practitioner to join our team in Woodbridge, New Jersey. This full-time, onsite role will empower you to take a central role in delivering high-quality primary care. You will work independently with patients while also collaborating closely with physicians and a multidisciplinary healthcare team. Our goal is to provide equitable, value-based primary care to adults on Medicare, ensuring they remain healthy, happy, and out of the hospital.

Work Environment

The typical work setting for this position is an assisted living facility in Woodbridge, NJ. You will be part of a collaborative care team comprised not only of fellow Nurse Practitioners and physicians but also medical assistants, dedicated medical scribes, and care managers. Together, you will work to coordinate patient care effectively while leveraging partnerships with a network of elite specialists and hospitals to provide specialty and acute care services when required.

Key Responsibilities

• Deliver exceptional primary care during office visits, addressing both routine and acute health issues.
• Conduct Annual wellness visits and health risk assessments with a comprehensive and detail-oriented approach.
• Coordinate care with various providers, specialists, and testing facilities to ensure optimal patient outcomes.
• Lead population health initiatives, ensuring all eligible patients receive evidence-based screenings, such as bi-annual breast cancer screenings.
• Assist the care team by performing phone triage and conducting patient outreach as needed.
• Educate patients on their health concerns, care plans, and treatment options, effectively guiding them through their healthcare journeys.
• Engage in promotional activities that enhance the visibility of our services in the community.
• Participate in home visits for patients who require personalized assessments or interventions.
• Report directly to the Medical Director while working closely with operational leadership, contributing to both clinical and administrative functions.

Patient Population / Client Focus

Your primary focus will be on older adults enrolled in Medicare. The aim is to maintain their health and well-being while enabling them to lead fulfilling lives. You will work directly in their communities, ensuring care is accessible and tailored to their individual needs. This role is particularly rewarding as you will have the opportunity to develop long-lasting relationships with your patients, which significantly contributes to their overall health outcomes.

Qualifications

• Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or completion of an accredited Nurse Practitioner Certificate Program.
• National certification from a recognized entity such as AANP or ANCC as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner.
• An active and non-probationary Nurse Practitioner license in the state of New Jersey.
• Possession of an active DEA license.
• Practical experience in primary care settings, with a focus on internal medicine, geriatrics, or family medicine.
• Proficiency in HCC documentation, ICD-10 coding, and completion of health risk assessments.
• A team-oriented mindset with a passion for innovative, population health-focused care.
• Bilingual proficiency is a plus, enhancing communication with a diverse patient population.

Preferred Experience

Experience in a similar healthcare setting, especially with older adults, is preferred. A background in value-based care delivery models will allow you to contribute more effectively to our innovative approach to patient care. Familiarity with care coordination practices and the ability to manage complex patient cases will be beneficial.

Compensation and Benefits

We offer competitive compensation in the range of USD 120,000 to 160,000 annually, commensurate with experience and expertise. Our comprehensive benefits package includes:
