Work Environment

Key Responsibilities

• Conduct thorough assessments to identify acute and chronic health conditions among residents, ensuring timely diagnosis and intervention.
• Design and implement personalized treatment plans, collaborating closely with other healthcare professionals to order diagnostic tests and medical imaging when necessary.
• Prescribe medications and recommend therapeutic interventions to manage patients' health effectively.
• Educate residents and their families about health management, lifestyle modifications, and preventive care measures, promoting overall wellness.
• Maintain and update medical records accurately, ensuring compliance with legal standards and facilitating seamless communication among the care team.
• Participate in quality improvement projects to enhance care protocols and patient satisfaction within the facility.
• Stay informed about current healthcare trends and regulatory changes to provide compliant and state-of-the-art care.

Patient Population / Client Focus

The primary patient population consists of elderly residents with diverse medical histories, often including complex chronic conditions requiring attentive management and individualized care approaches. You will play a critical role in enhancing their quality of life and supporting their healthcare journey through effective and proactive health interventions.

Qualifications

• Current certification as a Nurse Practitioner, with an active state nursing license from New Jersey.
• A Master's Degree in Nursing from an accredited institution is essential.
• At least three years of clinical experience in a relevant specialty, preferably involving primary care or geriatric nursing.
• Outstanding communication and interpersonal skills to foster trusting relationships with patients, families, and team members.

Compensation and Benefits

This role offers a competitive annual salary ranging from USD 150,000 to 180,000, commensurate with your experience and qualifications.
