Work Environment

The position is primarily onsite at an assisted living facility, providing an intimate working environment where you will interact closely with patients and their families. The facility houses residents requiring assorted levels of care, from those needing routine check-ups to individuals with complex health conditions. The role includes standard hours from 9:00 a.m. to 6:00 p.m., with some evenings extending to 8:00 p.m. and weekend community outreach participation required at times. This setting allows for the building of strong relationships with both patients and their families, enhancing both care delivery and community trust.

Key Responsibilities

• Conduct in-depth health assessments and screenings for residents, utilizing diagnostic tools and methodologies appropriate to geriatric care.
• Diagnose and manage both acute and chronic illnesses, ensuring the implementation of preventive healthcare measures and health promotion activities.
• Work collaboratively with multidisciplinary teams and community organizations to develop and facilitate health education initiatives, maximizing community engagement.
• Champion patient rights through advocacy, creating a culturally responsive healthcare environment that prioritizes individual needs and healthcare access.
• Coordinate referrals and ensure continuity of care through diligent follow-up with healthcare providers and specialists.
• Maintain comprehensive and precise medical records in accordance with healthcare regulations, including HIPAA compliance.

Patient Population / Client Focus

This role primarily serves residents of the assisted living facility, many of whom may have chronic health issues or complex medical needs. As such, you will utilize your specialized training to promote healthy aging, effectively manage medication regimens, and perform routine check-ups. You will work with diverse patients spanning various cultural backgrounds, providing care that respects and incorporates their values and beliefs into treatment plans. You will also be instrumental in identifying social determinants of health that affect your patients and strategizing ways to address these barriers.

Qualifications

• Possession of current Nurse Practitioner certification and active New Jersey nursing license.
• A Master's degree in nursing from an accredited institution is mandatory.
• Demonstrable experience in a clinical setting, particularly in community health, geriatrics, or public health.
• Exceptional interpersonal and communication skills, with proficiency in engaging and building rapport with diverse patient groups.

Preferred Experience

Compensation and Benefits

The salary for this position is competitive, ranging between $180,000 and $200,000 annually, dependent on qualifications and years of experience.
