Work Environment

Your role will be predominantly onsite at our assisted living facility, where you will collaborate with a team of healthcare professionals dedicated to community health. You will work from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m. Participation in weekend community outreach events or health fairs may also be requested, allowing you to engage directly with the community and further our health education efforts.

Key Responsibilities

• Conduct thorough health assessments and screenings for individuals and families, identifying specific health needs and concerns.
• Diagnose and manage both acute and chronic health conditions, including providing preventive care and health promotion advice tailored to the community's needs.
• Collaborate with local organizations and stakeholders to design and implement effective health education programs, enhancing community health literacy.
• Deliver culturally competent care while advocating fiercely for patients' rights to access essential healthcare services.
• Coordinate referrals and facilitate follow-up care to ensure seamless continuity of care for all patients.
• Maintain comprehensive and accurate medical records in compliance with regulatory standards, utilizing our electronic health record (EHR) system for efficient documentation.
• Engage actively in community outreach initiatives and health fairs, offering health screenings and promoting health awareness.
• Stay informed of community health trends, evidence-based practices, and relevant healthcare policies to ensure best practices are followed at all times.

Patient Population / Client Focus

Your primary focus will be the residents of the assisted living facility, as well as the broader Edison community, including underserved populations. You will leverage your clinical expertise to address various health concerns, with a strong emphasis on promoting preventive care and chronic disease management.

Qualifications

• Active state nursing license in New Jersey and current Nurse Practitioner certification.
• A Master’s degree in nursing from an accredited program is mandatory for this role.
• A minimum of 3-5 years of relevant clinical experience, ideally in community health or public health settings.
• Proficient interpersonal and communication skills, demonstrating an ability to engage effectively with diverse populations.

Preferred Experience

• Previous experience working in an assisted living facility or within community health programs.
• Demonstrated commitment to social justice initiatives and health equity advocacy is highly desirable.

Compensation and Benefits

We offer a competitive salary range of $180,000 to $200,000 per year, commensurate with your experience and qualifications.
