The Healthy Horizons Clinic is seeking dedicated and compassionate Full-Time Nurse Practitioners to join our dynamic Urgent Care team. In this pivotal role, you will be essential in diagnosing, treating, and managing various acute health conditions, while also promoting preventive healthcare practices. As an advanced practice registered nurse, you will play a critical role in ensuring our patients receive the highest quality of care in a supportive and patient-focused environment.

Work Environment

This role is based onsite at our Urgent Care Outpatient Office, with regular working hours set at 40 hours a week, typically from Monday to Friday until 5:00 PM. You will collaborate closely with fellow healthcare professionals in a team-oriented atmosphere, encouraging communication and sharing best practices to enhance patient experiences and outcomes.

Key Responsibilities

• Conduct thorough physical examinations and evaluate patients’ medical histories.
• Order and interpret diagnostic tests including blood tests and imaging studies to inform treatment plans.
• Diagnose acute illnesses and conditions, providing timely and effective treatment.
• Develop and propose treatment plans for both chronic and infectious diseases, taking into account patients' individual needs.
• Prescribe medications and provide education on their proper usage to ensure optimal patient health outcomes.
• Maintain meticulous patient records, documenting care plans, treatment progress, and outcomes.
• Educate patients and their families about health conditions, prevention strategies, and wellness practices.
• Support and contribute to research and continued professional development initiatives within the clinic.

Patient Population / Client Focus

Our Nurse Practitioners will work with a diverse patient population at Healthy Horizons Clinic, ranging from pediatric to geriatric patients. You will focus on providing urgent care services that address a variety of conditions, ensuring that each patient receives personalized care tailored to their unique health situations. Your expertise will help patients navigate their healthcare journey, fostering long-lasting relationships built on trust and respect.

Qualifications

• A valid New York State Nurse Practitioner license is required.
• A Master's degree in Nursing or a related field, with specialization as an Adult Nurse Practitioner (ANP) or Family Nurse Practitioner (FNP) is mandatory.
• DEA registration is essential for prescribing medications.
• Exceptional communication skills and a strong ethical framework.
• Team-oriented mindset with a positive attitude and a dedication to patient-centered care.

Preferred Experience

We value diverse experience in our candidates. As such, the ideal Nurse Practitioner will have:

• At least two years of proven experience in various care settings including clinics, hospitals, emergency rooms, or urgent care centers.
• Expertise in primary, acute, and chronic care practices.
• Familiarity with relevant health regulations and patient care standards.
• Bilingual candidates are a plus, enhancing our ability to serve our diverse patient base.

Compensation and Benefits
