Work Environment

This role is based in our outpatient office in Amityville, where a collaborative care team awaits you. You will work alongside physicians, other nurse practitioners, physician assistants, and support staff including medical assistants and dedicated medical scribes. This multidisciplinary team emphasizes collaboration and care coordination, allowing you to focus on delivering high-quality primary care services to our patients in a supportive environment.

Key Responsibilities

• Deliver exceptional primary care tailored to meet the specific health needs of each patient.
• Conduct comprehensive office visits addressing both routine and acute health issues.
• Administer annual wellness visits and detailed health risk assessments, ensuring holistic evaluations that prioritize thorough and accurate documentation.
• Engage in care coordination, collaborating with other healthcare providers, specialists, and testing facilities to ensure seamless patient experiences.
• Lead initiatives in population health management, promoting preventive measures such as evidence-based breast cancer screenings.
• Assist the care team through phone triage, outreach efforts, and patient education regarding their health conditions and treatment plans.
• Participate in community promotional activities that reflect our commitment to patient health and engagement.
• Conduct home visits as necessary to meet the needs of patients with mobility challenges.
• Report directly to the Medical Director, fostering a close working relationship with the operational leadership team.
• Complete additional duties as assigned to support clinic operations.

Patient Population / Client Focus

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or completion of an accredited Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through either the AANP or ANCC.
• Possession of an active, non-probationary state Nurse Practitioner license and an active DEA license.
• Demonstrated experience in a primary care setting, particularly in internal medicine, geriatrics, or family medicine.
• Proficient in HCC documentation, ICD-10 coding, and health risk assessments.
• A deep passion for collaborative teamwork and a commitment to engaging in cross-functional partnerships.
• A desire to be part of an innovative care model focused on evidence-based population health strategies.
• Bilingual proficiency is highly desirable for effective patient communication.

Preferred Experience

• Hands-on experience with Electronic Health Records (EHR) systems, enhancing patient care coordination.

Compensation and Benefits
