Key Responsibilities

• Conduct thorough assessments, including taking comprehensive patient histories, performing physical examinations, and interpreting diagnostic tests (e.g., CBCs, EKGs, and radiographs).
• Formulate and implement patient-specific treatment plans, integrating best practices and evidence-based guidelines to address both acute and chronic health conditions.
• Prescribe medications, ensuring they align with patient needs, safety guidelines, and cost-effectiveness while complying with state regulations.
• Provide immediate care for acute healthcare challenges, such as infections, injuries, and other health concerns, taking swift action to stabilize and treat patients effectively.
• Collaborate with a multidisciplinary team, including physicians, nurses, and support staff, to facilitate seamless patient care and foster open communication among team members.
• Educate patients and their families on health maintenance, disease prevention, and management of existing health issues, empowering them to take control of their health outcomes.
• Analyze clinical data and patient responses to modify treatment plans as necessary, always striving for optimal patient outcomes.

Patient Population / Client Focus

The primary focus of this role is on the unique needs of residents within an assisted living facility. You will work with a diverse group of patients, including those with complex health conditions, chronic illnesses, and varying degrees of mobility. Your work will directly influence their quality of life, requiring a compassionate approach to care that encompasses both physical and emotional support.

Qualifications

• Active, unrestricted Nurse Practitioner license in New York State.
• Master of Science in Nursing or Doctor of Nursing Practice (DNP) from an accredited institution.
• National certification as a Nurse Practitioner through AANP or ANCC.
• US work authorization.
• Strong interpersonal skills with the ability to work effectively in a team-oriented environment.
• Excellent communication skills to interact constructively with patients, families, and team members.

Preferred Experience

• Prior experience in a long-term care or assisted living environment is preferred but not required.
• Experience working with older adults or individuals with chronic health conditions.
• Familiarity with electronic health records (EHR) and clinical documentation.

Compensation and Benefits

We offer a competitive salary range of USD 150,000 to 160,000 annually , commensurate with experience and caseload.
