Work Environment

This position is based at our outpatient office in Rockaway Park, where you will work a standard schedule from 9:00 a.m. to 6:00 p.m. with occasional evening clinics until 8:00 p.m. Community outreach and health fair participation on weekends may also be needed, as part of our commitment to engaging with the population we serve.

Key Responsibilities

• Perform comprehensive health assessments to evaluate the physical, psychological, and social needs of patients in our community.
• Diagnose acute and chronic health conditions, creating effective management plans that include preventive care and health education for lifestyle modifications.
• Collaborate with local organizations to develop and implement educational programs that inform the community about prevalent health issues and ways to address them.
• Advocate for patient rights and needs, ensuring culturally sensitive care while coordinating referrals and follow-up services to maintain continuity in care.
• Document patient interactions accurately and comprehensively, fulfilling all regulatory compliance requirements while maintaining patient confidentiality.
• Engage in community outreach initiatives and health fairs to provide vital health screenings and resources to the public.
• Regularly review community health trends, staying informed about evidence-based practices and adapting them to enhance our service delivery.

Qualifications

• Current certification as a Nurse Practitioner.
• An active and unrestricted nursing license in New York State.

Compensation and Benefits

We offer a competitive salary range of $180,000 to $200,000 annually, commensurate with experience and qualifications.
