Work Environment

This position is onsite and involves making house calls to patients within the Islip area. Key Responsibilities

• Conduct comprehensive assessments by evaluating patients’ health histories and current medical conditions, and develop tailored treatment plans that align with best practice guidelines.
• Prescribe medications and implement treatment regimens in accordance with established medical protocols and New York state regulations.
• Perform minor surgical procedures such as suturing and wound care, and provide assistance during major surgeries when necessary.
• Educate patients and their families about health management, chronic disease prevention, and promote lifestyle changes that enhance overall well-being.
• Maintain accurate and detailed patient records in compliance with healthcare regulations, ensuring patient confidentiality and integrity of information.

Patient Population / Client Focus

As a Nurse Practitioner at Nurturing Hands, you will be entrusted with providing care to a diverse patient population, including individuals with chronic illnesses, elderly patients, and health-conscious individuals seeking preventive care. You will build robust relationships with patients, understanding their unique needs and health goals, which is vital to delivering effective and personalized care.

Qualifications

• Board certification in Family Medicine or Internal Medicine.
• Active, unrestricted nurse practitioner license in New York state.
• Master of Science in Nursing, Doctor of Nursing Practice, or completion of an accredited NP Certificate Program.
• Must possess valid U.S. work authorization.
• Exemplary critical thinking and analytical skills with a patient-centric approach to care.
• Exceptional communication abilities, both written and verbal, to foster positive relationships with patients and families.

Preferred Experience

• Experience in a concierge medicine or primary care setting is strongly preferred.

Compensation and Benefits
