Work Environment

This position is based onsite at an Assisted Living Facility in Amityville, New York, which serves a diverse population of seniors and individuals requiring varying levels of care. Our facility is designed to promote a nurturing and supportive atmosphere where staff and patients collaborate towards better health outcomes. A typical work schedule ranges from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m., and participation in weekend community outreach efforts may be required. You can expect an average of 40 hours of work per week.

Key Responsibilities

• Conduct thorough health assessments and screenings to identify the needs of individuals and families in the community.
• Diagnose and manage a range of acute and chronic medical conditions, providing preventive care and health education to empower patients.
• Collaborate with community groups and healthcare stakeholders to design and implement effective health education programs.
• Advocate for culturally sensitive care, ensuring equitable access to healthcare services for all patients.
• Coordinate referrals and manage follow-up care, ensuring smooth transitions and continuity of care for patients.

Patient Population / Client Focus

The Nurse Practitioner will primarily serve the elderly and individuals with chronic conditions in our Assisted Living Facility. Qualifications

• Current certification as a Nurse Practitioner.
• Active nursing license in the state of New York.
• A Master's degree in Nursing from an accredited institution.
• Minimum of 3 years of clinical experience, preferably in community health, geriatrics, or public health.
