Work Environment

This position is onsite, with responsibilities primarily focused on home care settings. Key Responsibilities

• Conduct thorough assessments to diagnose and treat acute healthcare problems, including illnesses, infections, and injuries.
• Prescribe and manage medications, ensuring choices are based on efficacy, safety, and cost-effectiveness, adhering to legal and regulatory standards.
• Design and implement interventions aimed at modifying patient behaviors that contribute to health risks, promoting healthier lifestyle choices.
• Educate patients and their families about self-management strategies for chronic and acute illnesses, personalizing instructions to align with their individual circumstances and health literacy levels.
• Utilize Electronic Health Records (EHR) effectively to document patient interactions, treatment plans, and health progress in compliance with organizational and legal requirements.
• Order, perform, and interpret diagnostic tests such as complete blood counts (CBCs), electrocardiograms (EKGs), and radiographs (x-rays) as part of the comprehensive evaluation process.
• Analyze patient histories, signs and symptoms, and diagnostic information to formulate accurate diagnoses and develop corresponding treatment plans based on established guidelines.

Patient Population / Client Focus

As a Nurse Practitioner at Positive Pals, you will serve a diverse patient population encompassing individuals across all age groups. Your role will involve caring for patients with varying health backgrounds, including those with chronic diseases, mental health issues, and acute medical conditions. Your ability to adapt care to the unique circumstances of each patient will be essential in achieving optimal health outcomes.

Qualifications

• A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED).
• Current certification as a Nurse Practitioner from the NYSED.
• Certification in Basic Life Support (BLS) from the American Heart Association (AHA), obtained during facility orientation or before starting your role.
• Demonstrated clinical knowledge and the ability to manage complex cases effectively.

Preferred Experience

Previous experience in home care, primary care, or a hospital outpatient setting is preferred. Familiarity with EHR systems and a strong understanding of chronic disease management can enhance your effectiveness in this role.

Compensation and Benefits
