Tranquil Mind Health Clinic is seeking a dedicated and compassionate Advanced Psychiatric Nurse Practitioner to join our team in Hempstead, New York. Work Environment

The position is full-time and requires onsite presence to provide home care services. Key Responsibilities

• Conduct in-depth psychiatric evaluations to accurately diagnose mental health disorders, utilizing a combination of clinical interviews and standardized assessment tools.
• Develop, implement, and monitor individualized treatment plans incorporating evidence-based practices tailored to address each patient's unique needs and circumstances.
• Prescribe and manage psychotropic medications, assessing their effectiveness and making necessary adjustments based on ongoing evaluations and patient feedback.
• Provide tailored psychotherapy and counseling services through modalities such as cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT).
• Engage in multidisciplinary collaboration, communicating effectively with other healthcare professionals to coordinate comprehensive and cohesive patient care.
• Educate patients and their families about mental health disorders, treatment options, and self-management strategies, empowering them to play an active role in their recovery.
• Maintain accurate documentation of patient progress, treatment efficacy, and ongoing symptom changes to inform future care decisions.
• Schedule and conduct follow-up appointments to evaluate treatment success, address patient concerns, and adjust care plans as necessary.
• Participate in regular interdisciplinary team meetings to contribute insights and strategies that enhance patient care across the clinic.

Patient Population / Client Focus

You will serve a diverse patient population, including adolescents, adults, and elderly individuals experiencing various mental health conditions such as depression, anxiety, bipolar disorder, and PTSD. Qualifications

• A Bachelor’s degree in Nursing, with a Master’s degree in Psychiatric Mental Health Nursing preferred.
• Active and unrestricted Nurse Practitioner license in New York State.
• Certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) is required.
• Strong communication and interpersonal skills, with the ability to work effectively in a collaborative, team-oriented environment.
• Demonstrated expertise in psychiatric evaluation and treatment methodologies.

Preferred Experience

• Previous experience in a psychiatric or mental health setting is highly advantageous.
• Familiarity with electronic health record (EHR) systems to facilitate documentation and data management.

Compensation and Benefits

We offer a competitive salary range of $200,000 to $250,000 annually, commensurate with experience and qualifications. In addition, our benefits package includes:
