Work Environment

You will be working onsite at our outpatient office located in Bayside, New York. The collaborative atmosphere promotes teamwork and innovation, where healthcare professionals share knowledge to improve patient care. The facility is designed to support efficient workflows while ensuring patient comfort. Your average work week will comprise approximately 40 hours, primarily Monday through Friday from 9:00 a.m. to 6:00 p.m., with the flexibility to extend into evening clinics until 8:00 p.m. on occasions. Participation in community outreach events or health fairs during weekends is also encouraged.

Key Responsibilities

• Conduct comprehensive health assessments and screenings for patients, utilizing evidence-based practices to identify potential health issues.
• Diagnose and manage both acute and chronic medical conditions, tailoring care plans to meet individual patient needs while emphasizing preventive care and health promotion.
• Collaborate with local community organizations to develop and facilitate health education initiatives, empowering patients and raising awareness about health resources.
• Advocate for culturally sensitive care, ensuring patients understand their rights and have access to the necessary health services.
• Coordinate referrals and follow-up care, fostering continuity of care through effective communication with other healthcare providers.
• Maintain accurate and detailed medical records in compliance with regulatory standards, ensuring a seamless patient experience and adherence to best practices.
• Engage actively in community health initiatives, providing screenings and educational workshops that address public health concerns.
• Stay informed on current health trends, policies, and best practices, integrating this knowledge into patient care and community outreach efforts.

Patient Population / Client Focus

This position will focus on underserved individuals and families in the Bayside area, a diverse community with varied health needs. As a Nurse Practitioner, you will be pivotal in addressing social determinants of health, particularly for those who may face barriers to healthcare access. Your role will involve building trust with patients from different cultural backgrounds, providing care that respects their unique values and circumstances.

Qualifications

• A current certification as a Nurse Practitioner, indicating proficiency and commitment to the nursing profession.
• An active state nursing license to practice, certified to operate within New York.
• A Master's degree in nursing from an accredited institution, equipping you with advanced clinical knowledge.
• Minimum of 3 years of relevant clinical experience, particularly within community health or public health settings, is crucial for this role.
• Excellent interpersonal and communication skills, capable of effectively engaging with a diverse patient population.

Preferred Experience

While 3 years of clinical experience is required, those with 5 or more years, particularly in settings focused on community health, will find their expertise especially valued. Experience working with diverse populations and a background in implementing community health outreach programs will be advantageous.

Compensation and Benefits
