As a Nurse Practitioner working in a home care capacity, you will engage directly with patients in their homes, providing care that is both thorough and empathetic. You will collaborate closely with fellow healthcare professionals to discuss ongoing patient care strategies and improve clinical outcomes.

Key Responsibilities

• Conduct thorough assessments and physical examinations of patients in their home environment, gathering detailed medical histories to inform care strategies.
• Diagnose and manage both acute and chronic conditions, utilizing critical thinking and evidence-based practices to create individualized treatment plans.
• Order, interpret, and follow up on diagnostic tests effectively to provide a clear picture of patient health and adapt treatment plans accordingly.
• Prescribe medications and manage various therapeutic interventions within your scope of practice, ensuring patient safety and effective treatment.
• Provide comprehensive education and counseling to patients and their families regarding health management, lifestyle changes, and preventive care measures.
• Efficiently document all patient encounters and care activities within our electronic health record system to maintain clear, accurate records.
• Participate in interdisciplinary team meetings, contributing to holistic patient care and collaborating with physicians and other healthcare providers.
• Ensure adherence to hospital policies and procedures, as well as compliance with relevant state and federal healthcare regulations.
• Engage in ongoing quality improvement initiatives and professional development to maintain high standards of care and stay informed on best practices.

Patient Population / Client Focus

Your primary focus will be on providing care to patients with varying medical needs, including those requiring chronic disease management, preventive care, and rehabilitation support. Understanding the unique needs of individuals in their home settings is crucial to successfully fostering health and improving quality of life.

Qualifications

• A Master's or Doctoral degree in Nursing from an accredited Nurse Practitioner program.
• Valid and unrestricted Nurse Practitioner license in the State of New York.
• Board certification as a Nurse Practitioner in a relevant specialty such as Family Nurse Practitioner (FNP) or Adult Gerontology Nurse Practitioner (AGNP).
• A DEA license with prescriptive authority is mandatory.
• A minimum of 2 years of clinical experience in a hospital or acute care setting is preferred.
• Proficient in using electronic health record (EHR) systems and maintaining clinical documentation.
• Exceptional clinical judgment along with strong communication and organizational skills.
• A capability to work both independently and within a multidisciplinary team, demonstrating effective collaboration.

Preferred Experience

• Experience in home care or community health settings is highly valued.
• Prior involvement in quality improvement projects will be an advantage.
• Knowledge of Bronx demographics and community health challenges.

Compensation and Benefits
