Behavioral Health Case Manager

Job Summary Coordinates and supports mental health care for patients enrolled in Collaborative Care Management (CoCM) by providing evidence‑based, short‑term interventions, case management, and acting as a core member of the CoCM team, which includes the primary care provider and a psychiatric consultant. Responsibilities Supports and closely coordinates mental health care with the patient’s primary care provider, with recommendations from a psychiatric consultant. Screens and assesses patients for common mental health needs, including regularly administering assessments to monitor progress and treatment intervention efficacy. Monitors patients via telephone, virtual outreach, and in‑person as needed for changes in clinical symptoms, medication treatment adherence, side effects, and other concerns, and communicates findings to the Primary Care Provider. Provides brief evidence‑based interventions such as Behavioral Activation, Problem Solving Techniques, Motivational Interviewing, and other skills appropriate for primary care settings. Provides and coordinates referrals to treatment or psychosocial needs as clinically indicated. Participates in regularly scheduled (weekly) systematic case reviews with the psychiatric consultant. Communicates recommendations to the Primary Care Provider for patients not improving as expected. Tracks and documents patient follow‑up, clinical outcomes, and treatment recommendations in an EPIC‑based registry. Completes relapse prevention plans with patients in remission for their mental health needs. Qualifications Minimum Qualifications Bachelor’s Degree in social work, psychology, counseling, nursing or a related mental health field. Two years of mental health/behavioral health experience or a related setting. Formal education or specialized training in behavioral health (e.g., social work, nursing, psychology). Preferred Qualifications Master’s Degree in social work, psychology, counseling, nursing or a related mental health field. Demonstrated knowledge and experience in a behavioral health field. Ability to work effectively with people from a broad range of backgrounds. High degree of emotional intelligence and empathy towards others. Skill in using logic and reasoning to evaluate solutions and approaches. Knowledge of evidence‑based interventions such as Motivational Interviewing, Problem Solving Techniques, and/or Behavioral Activation. Physical Demands Lift and carry up to 25 lbs. Frequent sitting/standing and frequent keyboard use. May be required to kneel, bend, squat, and perform CPR.
