Description:
Provide patient care in home, and assisted living environments.
Key Responsibilities:
- Provide wound care, including cleaning, dressing, and monitoring for infection.
- Teach patients and their families how to manage care, administer medications, and perform necessary health maintenance in the home.
- Collaborate with physicians, therapists, and other members of the care team. Report changes in patient condition to the appropriate personnel.
- Maintain accurate and complete documentation of patient assessments, treatments, and progress.
- Assess the home environment for safety and make recommendations for improvement.
Qualifications:
- Completion of an accredited LPN/LVN program, and a current valid state LPN or LVN license.
- Clinical skills and experience
- Critical thinking and problem-solving skills
- Excellent communication skills to interact with patients, families, and colleagues effectively.
- Proficiency in various wound care techniques, products, and healing processes.
- Experience in PICC lines, midlines, and peripheral IV care and management.
- Ability to work collaboratively with a multidisciplinary healthcare team.
- Organizational and time-management abilities.
- Physical stamina, as the job can involve lifting and standing for long periods.
