LPN – Home Health

Schedule: 8 hours

Assignment Details:

• Contract Length: 13 weeks
• Guaranteed Hours: 40

Requirements:

• Current nursing license in the practicing state
• Valid driver's license
• Current CPR certification
• Two years of experience as an LPN in a clinical setting, preferably in a home health or hospice setting
• Proficiency in using electronic medical charting and time-keeping software (Homecare Homebase)

Roles & Responsibilities:

• Provide nursing services under the direction of a registered nurse, assessing patients' physical, psycho-social, and environmental needs.
• Assist in planning, coordinating, and implementing patient care in collaboration with the RN.
• Communicate effectively with patients, families, and healthcare professionals and document care outcomes appropriately.
• Educate patients and families on self-care techniques and assess their responses to care.
• Maintain infection control measures to ensure safety for both staff and patients.
• Assist with specialized duties related to end-of-life care for hospice patients.
• Support with patient admissions, discharges, and transfers.

Compliance / Notes:

• Must participate in annual in-service and performance improvement programs.
• Reliable transportation and current auto liability insurance coverage are required.
• Must read, write, and speak fluent English.
