Job Summary

This is a travel/contract Registered Nurse Case Manager position at an acute care facility, focused on Care Coordination and Discharge Planning. The role operates within the Case Management department and requires active California state licensure upon start.

Key Responsibilities

• Perform Care Coordination and Discharge Planning in an acute care hospital setting
• Meet weekend rotation requirements to meet department needs
• Maintain patient ratios of 1:20

Required Experience

• Minimum 2 years of acute care hospital Case Manager experience performing Care Coordination and Discharge Planning - hospital experience is required

Education Requirements

• California (CA) state RN license required (must be licensed upon start date; pending license accepted during application process)
