In this role, you will work remotely while providing essential services through our Community Health Clinic. Key Responsibilities\n

• Conduct in-depth evaluations and assessments to understand the individual needs of children and families.

• Participate actively in the development and implementation of Individualized Family Service Plans (IFSP), tailoring interventions to meet the specific needs of each child.

• Monitor progress and outcomes as a collaborative member of a multidisciplinary team, contributing to ongoing assessments as children progress within their developmental milestones.

• Offer guidance and support to families around developmental challenges, providing evidence-based strategies to enhance their child’s growth.

• Utilize coaching models and Natural Learning Environment Practices to create interactive learning experiences for children.
\n
• Build rapport with children and families by fostering a trusting and supportive relationship that promotes a healthy learning atmosphere.

• Ensure compliance with all relevant state and federal regulations while maintaining the highest standards of service delivery and client confidentiality.

• Collaborate with other healthcare professionals to ensure holistic care and support for families.

Patient Population / Client Focus\n

The primary focus of this role is on young children, particularly those with disabilities or developmental delays. Qualifications\n

• Master's Degree or Doctorate in Social Work from an accredited program.
