Schedule: Full-Time, Day Shift What You'll Do Perform program-specific patient outreach, education, and recruitment. Consult with patients’ physicians and/or Medical Directors about high-priority patients and potential care management plans. Initiate contact with eligible patients to perform initial assessments and develop care management plans. Implement high-quality, cost-effective care plans using case management principles. Communicate effectively with patients, families, and physicians to build trust and engage patients in health promotion activities. Provide direct care coordination and encourage enrollment in disease and case management programs. Determine appropriateness of hospital admissions and disease management programs. Communicate with healthcare clinicians about patient care and follow-up plans. Establish a communication schedule for periodic contact with providers and patients. Attend and present case reviews at practice and care coordination meetings. Requirements Education: Bachelor's Degree in Nursing required, Master's Degree in Nursing preferred. Experience: Minimum 1–3 years of oncology care experience within the past five years required. Minimum 1 year of case management experience in an outpatient setting within the past five years required. Experience with complex patient populations (e.g., solid tumors, hematologic malignancies) strongly preferred. Health care reimbursement experience strongly preferred.Knowledge of insurance authorization processes and healthcare systems strongly preferred.
