The LVN provides individualized nursing care in collaboration with physicians, nursing staff, residents, families, and other departments to ensure optimal resident care. ResponsibilitiesConduct individualized patient assessments and prioritize data collection based on patients' immediate needs. Develop and implement personalized care plans in collaboration with the healthcare team. Administer medications and treatments as prescribed while ensuring compliance with policies. Document patient assessment findings, responses to interventions, and progress toward goals. Provide patient and family education tailored to individual needs. Initiate emergency protocols and provide care in emergency situations as required. Maintain confidentiality and adhere to safety and infection control standards. Participate in training and orientation of new staff as needed. Assist in direct patient care and respond promptly to call lights or requests for assistance. Required Experience / Certifications / LicensureValid California LVN license. Current BLS level CPR certification. Strong assessment and problem-solving skills. Ability to adapt, communicate clearly, and maintain positive relationships with staff and residents. Dependable and reliable with a strong attendance record.
