LVN / LPN - LTC/SNF

Job Description Job Summary: The LVN/LPN provides individualized nursing care in an LTC/SNF setting while working in collaboration with physicians, the healthcare team, residents, and families. Reporting to the DON, the LVN/LPN supports assessment, treatment, medication administration, documentation, and emergency response to promote safe, patient-centered outcomes. Responsibilities: Conduct individualized patient assessments and prioritize data collection based on immediate needs. Notify appropriate clinical leaders of abnormal findings and patient status changes. Develop and implement individualized plans of care in collaboration with the interdisciplinary team. Perform ordered treatments accurately and in a timely manner, including required bedside procedures. Administer medications according to policy/procedure, ensuring right patient, right medication, correct timing, and proper documentation. Initiate and document emergency resuscitation measures and respond promptly to urgent situations per protocols. Qualifications: Hold an active CA LVN/LPN license in good standing. Maintain current BLS/CPR certification. Demonstrate strong assessment skills and resourcefulness in emergency situations. Provide safe, competent direct patient care with the physical ability to walk, bend, lift, and perform manual skills. Communicate effectively in English, verbally and in writing, with professional interpersonal skills. Demonstrate reliability and professionalism, including confidentiality, infection control, and respectful teamwork.
