Behavioral Health Outpatient Clinician - Lewiston

Title: Outpatient Clinician

What You'll Do:

Assesses and identify the client's holistic health care needs/concerns as part of a family and community.
Provide psychosocial assessments for clients.
Develop individualized treatment plans for youth, adults, and families.
Implement patient care plan activities based on treatment goals towards independent living for the patient/family.
Serve as resource for staff education & training, participate in case conferences as needed.

What You'll Bring:

Master's degree from a school of social work accredited by the council on social work education.
Maine Board of Social Work Licensure: LCPC, LCPC-C, LCSW or LMSW-CC
One or more years of experience as a master's level social worker in a health care setting.
A valid Drivers License and reliable car with insurance.
