This is a full-time opportunity and this position will serve for our Ozaukee County $8,000.00 Sign on Bonus RN Care Manager Job Responsibilities: Completes discipline-specific initial and ongoing assessments. Conducts home visits and home assessments as needed, and consults w/ families and other supports for additional information as needed. Develops and maintains care plans, including members' long-term care and personal outcomes, progress towards past identified outcomes, member strengths, risks, natural and formal supports. Follow up on all services to ensure quality/coordination and effectiveness of services. RN Care Manager Job Requirements: Education: Associates degree in Nursing Required, Bachelors Degree preferred. Licensure: Wisconsin Licensed Registered Nurse Licensure: Eligibility for long-term care functional screener certification is required Licensure: WI Drivers License Experience: Required 2 years recent experience with older adults or persons with physical or intellectual disabilities in a nursing capacity Required Preferred 2 years of work in community nursing RN Care Manager Job Schedule: Monday to Friday 8:00AM – 4:30PM About Community Care Community Care is a local nonprofit with more than 40 years of experience helping older adults and adults with disabilities live as independently as possible within the community.
