Support and Service Coordinator - Children’s Long Term Support Program (CLTS)

Are you passionate about supporting children and youth with disabilities and helping families navigate complex systems and resources? The Support and Service Coordinator (SSC) role offers an opportunity to build meaningful relationships with youth and families while helping them access the services, supports, and resources they need to thrive in their homes and communities.

As an SSC, you will serve as a trusted partner and advocate for children, youth, and their families enrolled in Wisconsin’s Children’s Long-Term Support (CLTS) Waiver Program. You’ll help families identify their strengths, goals, and needs, connect them with resources, and coordinate individualized services that promote independence, inclusion, safety, and quality of life.

This is a relationship-driven role for someone who is   compassionate, organized, resourceful, culturally responsive, and comfortable advocating for others.

What You’ll Do

Build Relationships & Support Families

• Develop trusting, professional relationships with children, youth, families, and their support networks.
• Provide   family-centered, trauma-informed, strength-based, and culturally responsive   support.
• Maintain regular contact with families and youth through phone, virtual, community, and face-to-face interactions.
• Meet with youth and families in their homes and communities to better understand their needs, strengths, and goals.
• Support families in making informed decisions and exercising self-determination.

Coordinate Services & Create Opportunities

• Coordinate services and supports that address the developmental, emotional, physical, and functional needs of children and youth.
• Develop and implement individualized, family-centered service plans based on each youth’s unique needs and desired outcomes.
• Connect families with formal and informal supports, community resources, providers, schools, and other agencies.
• Promote meaningful inclusion and participation in the community and the youth’s home environment.
• Coordinate benefits and services to help ensure families receive appropriate, high-quality supports regardless of funding source.
• Authorize eligible supports and services available through the CLTS Program.
• Provide transition support when youth enroll in or transition out of the CLTS Program.

Advocate & Problem-Solve

• Advocate alongside youth and families to help them access services, resources, and opportunities.
• Empower families with information and tools so they can independently navigate systems and access supports.
• Identify children and youth who may be vulnerable or at higher risk and provide enhanced care coordination when needed.
• Collaborate with schools, providers, county programs, community organizations, and other agencies to address complex needs.
• Respond professionally and effectively to crises and critical incidents.

Promote Health, Safety & Well-Being

• Assess and periodically reassess the child or youth’s health, safety, and functional needs.
• Help identify and address barriers that may affect a youth’s ability to remain safe, healthy, and included in their community.
• Fulfill mandated reporter responsibilities, including reporting concerns regarding a child or youth’s mental health, safety, or well-being as required by Wisconsin law and program requirements.

Stay Organized & Accountable

• Maintain accurate, timely, and comprehensive documentation in accordance with CLTS and agency requirements.
• Complete required paperwork in a   strength-based and youth- and family-centered manner .
• Maintain program functional and financial eligibility requirements.
• Complete required CLTS, functional screening, mandated reporter, and other applicable training.
• Follow CLTS policies, procedures, and professional standards.

We’re looking for someone who brings both   compassion and strong organizational skills   to their work. Successful SSCs are comfortable building relationships with diverse families, navigating complex systems, solving problems, and advocating for children and youth.

You’ll bring:

• Knowledge of or experience working with children, youth, or young adults with disabilities and/or complex support needs.
• A compassionate, patient, and nonjudgmental approach to working with individuals and families who may have experienced trauma or challenging life circumstances.
• The ability to remain calm, professional, and solution-focused during challenging or escalating situations.
• Strong communication, interpersonal, and relationship-building skills.
• The ability to advocate effectively while respecting youth and family choice, culture, values, and self-determination.
• Strong organizational and time-management skills, including the ability to prioritize and manage multiple responsibilities.
• The ability to work independently while also collaborating effectively with a multidisciplinary team.
• The ability to understand and follow written and verbal instructions, policies, and program requirements.
• Basic computer skills and proficiency with Microsoft Word and Excel.
• Sensitivity and respect for diverse cultural, ethnic, socioeconomic, and disability-related experiences and needs.

Collaboration & Work Environment

The SSC reports directly to the   CLTS Supervisor   and works collaboratively with a broad network of partners. This includes youth and families, formal and informal supports, service providers, funders, county staff, schools and Milwaukee Public Schools (MPS), disability-serving organizations, community agencies, and other professionals involved in supporting the youth.

The role includes participation in team meetings, staffing, consultations, required training, and ongoing professional development. SSCs may also provide coverage and support for colleagues and their families when needed.
