Position Summary:
The Liberty House Case Manager is a key member of the Transitional Housing team and plays a critical role in supporting individuals and families toward housing stability, self-sufficiency, and long-term independence.The Case Manager carries a caseload of individuals, families, or a combination of both and provides intensive, community-based case management services. This role requires comfort with traveling throughout New York City via public transportation, entering participant homes independently without support, conducting home visits and unit inspections, facilitating supportive groups, responding to crisis situations, and working closely with justice-involved, vulnerable, and diverse populations.The Case Manager works in close partnership with all Transitional Housing, justice-involved, and social service staff, and must demonstrate flexibility, accountability, and strong professional judgment. Evening and weekend hours may be required to meet participant needs and program demands.
Competencies / Skills:
Client Engagement and Relationship Building: Builds trust and rapport with participants from diverse cultural, racial, gender, and socioeconomic backgrounds. Demonstrates empathy, active listening, cultural humility, and professional boundaries while offering supportive, goal-oriented guidance.
Assessment, Interviewing, and Service Planning: Demonstrates strong interviewing skills to assess participant strengths, needs, barriers, family relationships, community supports, cultural factors, medical follow-up, and service engagement. Completes comprehensive and ongoing assessments. Develops, updates, and documents individualized service plans in partnership with participants and interdisciplinary team members.
Case Coordination, Advocacy, and Referrals: Coordinates care and makes referrals as needed to internal and external services including housing, benefits, mental health, substance use treatment, medical care, employment, education, and legal services. Advocates for participants across complex systems including courts, probation/parole, ACS, behavioral health, and social services.
Group Facilitation and Skill Building: Facilitates or co-facilitates groups that support case management goals, including but not limited to financial literacy, self-care, art-based groups, life skills, and peer support groups. Uses groups to reinforce independence, emotional regulation, community connection, and permanency goals.
Documentation and Data Management: Maintains accurate, timely, and compliant documentation following Housing Plus documentation standards, templates, and procedures. Completes all required case notes, service plans, assessments, genograms, presentation forms, and progress updates in AWARDS within required timeframes (no later than 72 hours). Ensures timely collection of participant and supervisory signatures and maintains audit-ready files.
Crisis Intervention and Problem Solving: Responds appropriately to crises, assesses risk and safety concerns, implements de-escalation strategies, and coordinates emergency interventions as needed. Demonstrates calm judgment, flexibility, and ethical decision-making.
Professional Communication and Collaboration: Demonstrates professional verbal and written communication, including appropriate email etiquette with participants, colleagues, supervisors, and collateral providers. Presents cases clearly in team meetings, case conferences, and supervision, highlighting participant progress, tasks, strengths, and ongoing challenges.
Cultural Competence, Ethics, and Accountability: Works effectively with vulnerable populations impacted by trauma, incarceration, homelessness, behavioral health needs, and systemic inequities. Applies culturally responsive, trauma-informed, and ethical practices. Demonstrates reliability, time management, and accountability for outcomes.
Duties/Responsibilities:
- Conduct comprehensive intakes/discharges (as needed), ongoing assessments, and structured interviews to identify participant needs, strengths, and goals.
- Develop, document, and monitor individualized service plans in partnership with participants and interdisciplinary team members.
- Present participant case progress, strengths, challenges, and next steps during supervision, team meetings, and case conferences.
- Facilitate or co-facilitate supportive groups (e.g., financial literacy, self-care, art-based groups, support groups).
- Conduct bi-weekly home visits and unit inspections, including follow-up to ensure safety, cleanliness, and habitability.
- Support participants in building independent living skills to improve readiness for permanency and housing transition.
- Enter participant units independently and confidently in accordance with safety protocols.
- Coordinate with collateral contacts including, but not limited to, courts, probation/parole, mental health providers, substance use programs, medical providers, and social service agencies.
- Make referrals as needed and track follow-through and outcomes.
- Assist participants in obtaining identification and documentation, completing applications, and escorting participants to community appointments when necessary
- Provide crisis intervention, safety planning, and emergency coordination as needed.
- Occasionally, travel throughout NYC using public transportation for home visits, appointments, and community-based services.
- Maintain accurate, timely, and compliant documentation in AWARDS within 72 hours of all participant contact.
- Ensure all documentation follows agency templates and procedures and includes required signatures.
- Maintain organized, confidential, and audit-ready participant files.
- Work collaboratively with all Transitional Housing, justice-involved, and social service staff.
- Attend required program meetings, staff meetings, community meetings, training, and supervision.
- Work evenings or weekends as required to meet participant and program needs.
- Other tasks as assigned.
Required Skills/Abilities:
- Strong comfort working with highly vulnerable populations and adapting services based on participant needs to support success.
- Ability to travel independently via public transportation throughout NYC.
- Ability to safely and confidently conduct home visits and unit inspections without support.
- Experience with group facilitation, crisis intervention, and intensive case management.
- Strong interviewing, assessment, documentation, and follow-up skills.
- Knowledge of justice, housing, behavioral health, and social service systems.
- Proficiency in Microsoft Office, AWARDS, and virtual platforms.
- Professional written communication and email etiquette.
- Strong organizational skills and ability to manage competing priorities.
- Flexibility to work evenings and weekends as needed.
Education and Experience:
- Associate or Bachelor's degree preferred in social work, human services, or a related field.
- Minimum of 3 years of experience in social services or nonprofit settings, preferably in NYC; a Master's degree in social work or a related field may substitute for years of experience.
- Experience working with justice-impacted individuals, women experiencing homelessness, trauma, behavioral health needs, or substance use strongly preferred.
Additional Schedule Details:
- Standard work schedule is Monday–Friday, 9:00 AM–5:00 PM.
