Community Health Navigator: Access & Wellness Advocate
Hampton Roads Community Action Program
Hampton Roads Community Action Program in Newport News, VA seeks a Community Health Outreach Navigator to connect residents with healthcare services, public benefits, and food assistance while addressing social determinants of health.
You will perform needs assessments, provide referrals, support enrollment in Medicaid/Medicare and Marketplace plans, and collaborate with partners to improve care coordination and health outcomes.
