RN (Geriatrics) - Full-time

Job Summary:

We are seeking a compassionate and experienced RN (Geriatrics) - Full-time to join our team at Autumnwood Care Centre, a vibrant community dedicated to providing exceptional care and support to our residents. As a key member of our nursing staff, you will play a crucial role in delivering high-quality healthcare to our geriatric patients, working closely with our interdisciplinary team to ensure the best possible outcomes.

Responsibilities:

• Provide direct patient care, including assessment, planning, implementation, and evaluation of individualized care plans for geriatric patients with complex needs

• Collaborate with other healthcare professionals to develop and implement effective treatment plans, including medication management and wound care

• Monitor patients' conditions, reporting any changes or concerns to the healthcare team

• Educate patients and families on disease management, nutrition, and other health-related topics

• Develop and maintain positive relationships with residents, families, and colleagues to promote a culture of respect, empathy, and open communication

• Participate in quality improvement initiatives to continuously improve patient care and outcomes

• Maintain accurate and up-to-date records of patient care and communications
