Responsibilities
- Conduct initial evaluations and ongoing assessments of patients' physical therapy needs.
- Develop and implement individualized treatment plans to improve mobility, strength, balance, and functional abilities.
- Perform therapeutic exercises, gait training, and use of modalities as appropriate to patient goals.
- Monitor progress, re-evaluate plans of care, and document outcomes in the medical record.
- Educate patients and families on home programs, safety, and activity modification; coordinate care with the multidisciplinary team.

Requirements
- Doctor of Physical Therapy (DPT) or equivalent degree.
- Current physical therapy license or eligibility for licensure in the United States; ability to obtain licensure as required by state regulations.
- Experience in home health, skilled nursing, or hospital settings is preferred.
- Strong clinical assessment, documentation, and communication skills.
- Ability to travel between sites and work in a rural setting; flexibility to work across multiple care environments.
