Physicians Choice Home Health Physical TherapistPhysicians Choice Home Health is hiring a full-time or part-time/per diem Physical Therapist (PT) to join our Home Health team, serving patients in the South Bay of Los Angeles. In this role, you will provide high-quality physical therapy services to patients in the comfort of their own homes, helping them regain independence and improve their daily functioning. You will oversee a supporting Physical Therapy Assistant and a team of dedicated healthcare professionals who are committed to delivering exceptional patient-centered care.What You'll Do:Provide in-home physical therapy evaluations and treatmentsAssess patients' strength, mobility, balance, and ability to function safely at homeCreate and update individualized plans of care that support independence and recoveryHelp patients regain mobility and confidence in their home environmentEducate patients and caregivers on exercises, safety, and fall preventionDocument visits and evaluations in a timely mannerCollaborate with nurses, therapists, and the broader care team to support patient outcomesSupervise Physical Therapy Assistants (PTAs) and support their clinical developmentParticipate in care planning, case conferences, and discharge planning
