Job Description

Job Summary:

Responsible for providing comprehensive physical therapy services to home health patients, including evaluations, treatment planning, and therapy implementation. The role requires traveling to patient homes and coordinating with office meetings to ensure quality patient care.

Responsibilities:

• Conduct patient assessments and develop individualized treatment plans in accordance with physician orders.
• Perform therapy interventions during home visits, documenting progress and adjustments as needed.
• Maintain timely and accurate documentation of patient encounters within 24 hours.
• Travel up to 75 miles from the home clinic to deliver care efficiently across the territory.
• Attend periodic office meetings and ensure compliance with agency policies and procedures.
• Collaborate with healthcare team members to optimize patient outcomes and coordinate care.

Qualifications:

• Hold a valid state license in Physical Therapy with current licensure in hand.
• Have a minimum of 1 year of recent adult home health experience.
• Proficient in HomeCare HomeBase EMR system and familiar with OASIS assessments.
• Demonstrate reliable transportation, a valid driver's license, and agency-required liability insurance.
• Possess current CPR and BLS certifications.
• Willingness to travel up to 75 miles and work flexible shifts, including potential weekend coverage.
