RN Case Manager Provides direct nursing care and oversees home health services for clients. Collaborates with clients, families, and the healthcare team to develop and implement goal-focused care plans, considering quality measures and financial aspects of treatment. Provides skilled nursing care and clinical oversight to professional and unlicensed personnel. Builds and maintains respectful relationships. Reports to Clinical Supervisor. SCHEDULE Full-time position, Monday - Friday with an on-call rotation. Manages your own schedule during office hours. Travels to client homes to provide 1:1 client care, typically 1–3 hours per client. COVERAGE AREA Reports to the Mendota Heights, MN office and provides services in Northfield and surrounding cities. MAJOR RESPONSIBILITIES Reflect the company’s mission and values in developing and maintaining professional relationships. Perform initial and periodic assessments of clients to determine care needs, implement interventions, and evaluate effectiveness. Collaborate with physicians/non‑physician practitioners and the home health team to develop and update the client’s home health plan at least every 60 days or more often as needed. Serve as client advocate ensuring safety needs are met, assessing home environment, developing individualized abuse prevention plans, and reporting suspected abuse, neglect or financial exploitation. Meet documentation standards ensuring accuracy, timeliness, and compliance with payer/billing requirements. Provide education to clients, families, caregivers, licensed clinicians (as indicated), and unlicensed personnel. Provide direct care to clients of varying ages, acuity levels and diagnoses per the company’s policies, procedures, processes and the Minnesota Nurse Practice Act. Coordinate client care with family, caregivers, community resources, and other health care members. Provide clinical oversight, delegation and leadership to licensed clinicians and unlicensed personnel. Ensure effective financial management of the client’s home health services. Manage work schedule effectively, adapt to unexpected changes and participate in the on‑call process. PHYSICAL / ENVIRONMENTAL DEMANDS Client care is performed in individual client homes and may require transfers and lifting with adaptive equipment. Must be adaptable to a variety of environments and community settings. Must be able to drive or use public transportation in all types of weather. Weight moved: 50–100 pounds occasionally; 25–50 pounds frequently; 10–20 pounds constantly. QUALIFICATIONS Currently licensed as a Registered Nurse (RN) in the State of Minnesota without revocation, suspension or restrictions. Proof of negative TB testing: Mantoux skin test, single TB blood test or chest X‑ray. Access to a dependable automobile for travel to multiple company stops per day. Valid driver’s license and proof of current car insurance when using a personal vehicle for company business. U.S. Citizenship or evidence of a valid Alien Work Permit. Disclose any conviction or criminal history related to health services. Candidates with such convictions will not be hired. Pass initial and ongoing background studies and screenings, including those of the Minnesota Department of Health and the Federal Office of the Inspector General’s List of Excluded Individuals and Entities. SKILLS AND EXPERIENCE At least two (2) years’ experience in a health care facility or home care field. Demonstrates critical thinking, ability to follow standards and procedures, and make appropriate clinical judgments. Can work independently with minimal supervision, adapt to change, and manage multiple assignments while setting priorities. Ability to build and maintain professional relationships. Excellent written and oral communication skills; interpersonal communication and teaching skills. Basic computer skills and willingness to learn new software; experience with EMR and Microsoft Outlook preferred. Current CPR certification highly recommended. COMPENSATION Base salary range: $70,000 – $95,000 per year plus productivity, compensation and benefits.
