Care Companion/Social Worker Join the Carease Health team as a compassionate and dedicated Care Companion/Social Worker, providing emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities in Madison, WI. As a key member of our Collaborative Care team, you will make a direct impact on the lives of older adults, helping them feel heard, supported, and emotionally connected within their environment. About the Role As a Care Companion/Social Worker, you will: Visit designated facilities in accordance with the schedule set with your supervisor Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns Document visit notes and maintain detailed records of interactions and interventions after each visit Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP Qualifications Associates, Bachelor’s or Master’s degree in Social Work, Psychology, Counseling, or another mental health–related field Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings Strong communication, documentation, and relationship-building skills Education: Associate Degree Experience: 1 year Why Join Carease Health?
