Part-Time Nurse Practitioner – Transitional Care in San Antonio Flexible Schedule | Supplemental Income | Home-Based Patient Visits Trumen Physicians + Associates is seeking an experienced and compassionate Nurse Practitioner to join our Trumen Transitional Care Team (TTC) on a part-time, supplemental basis in San Antonio, TX. About the TTC Program: Trumen’s Transitional Care Program supports adults with acute or chronic conditions as they move from the hospital or skilled nursing facility back to their community setting—such as their home or assisted living. The goal is to prevent avoidable readmissions by ensuring a smooth, medically supported transition. As a TTC Provider, you will: * Travel to patients' homes shortly after discharge * Reconcile medications * Review and adjust treatment plans * Manage acute and chronic conditions * Communicate with patients’ family and primary care providers when needed You will work closely with a dedicated TTC Coordinator to build your monthly schedule based on your availability and travel preferences.
