Job Title Hospice RN, PRN Location Lisle, IL, USA (and surrounding areas) Position Overview We are seeking a compassionate Hospice Registered Nurse to support patients in the home setting. As part of our team‑based care model, you will collaborate to manage and deliver patient care plans. Key Responsibilities Provide hospice care and support to patients in Lisle, IL and surrounding areas. Collaborate with the care team to develop and implement individualized care plans. Document patient status and care actions in the electronic medical record (EMR) in a timely and accurate manner. Coordinate with physicians, therapists and other professionals to ensure comprehensive care. Educate patients and families about care plans and hospice services. Travel to patient locations, spending approximately 20% of your day driving. Self‑manage scheduling and workload to meet critical charting deadlines. Qualifications RN license in the state of employment. Associate degree or higher from an accredited nursing school. Minimum of two years of RN experience, with hospice experience required. Valid driver’s license and reliable transportation. Strong written and verbal communication skills. Proficiency with tablets, mobile phones and EMR software.
