Summary The Veteran Directed Care (VDC) program is a Home and Community-Based Care (H&CBC) service designed for Veterans who would otherwise qualify for nursing home care but require significant support to safely remain in their home or community setting. Through VDC, Veterans are allocated an individualized budget to direct their own services. The Registered Nurse plays a critical role in ensuring clinical appropriateness, program integrity, and quality oversight. Qualifications English Language Proficiency: Mandatory per 38 U.S.C. 7403(f) – may not serve in direct patient care unless proficient in basic written and spoken English. Graduate of a professional nursing school approved by an accrediting agency (ACEN or CCNE). Or individuals in a master’s level bridge program who have completed coursework equivalent to a bachelor's in nursing may apply for state licensure before completion. After obtaining a state license, candidates may be appointed on a temporary basis and later converted to a permanent appointment upon successful completion of the bridge program. Foreign nursing school graduates must possess a current, full, active, and unrestricted registration and a formal degree equivalency validation from a recognized equivalency evaluation (e.g., International Consultants of Delaware). Graduate Nurse Technician (GNT) Exception: Candidates meeting all basic education requirements but lacking licensure may be appointed at the entry step of the applicable grade on a 120‑day temporary appointment while pursuing licensure, extendable up to two years on a case‑by‑case basis. Grandfathering Provision: All employees in the VHA in the 0610 series performing duties under the qualification standard on 1/29/2024 are considered to have met all requirements for the grade held. Grade determination follows scope, education, and dimension criteria. Specific requirements for Nurse I–III levels are detailed in the online assessment (link provided). Preferred Experience: BSN or higher; recent (within 5 years) experience with processing clinical consults/referrals; clinical experience in acute or ambulatory care, patient triage, case management/care coordination; proficiency in EHR and MS Office tools; strong writing and verbal communication skills; nursing certification. Physical Requirements: Pre‑placement examination required to determine safe operation of a government vehicle. Must meet vision, hearing, and color discrimination standards as specified. Duties Provide care coordination and program monitoring for Veterans with high functional, medical, and psychosocial needs as the H&CBC Registered Nurse assigned to the VDC program. Ensure Veterans receive safe, high‑quality, and cost‑effective care while maintaining autonomy in their preferred care setting. Assess and determine clinical appropriateness of Veterans for enrollment in the VDC program. Apply and implement the Case Mix Index (CMI) to establish appropriate levels of care and budget allocations. Develop, review, and monitor individualized Veteran budgets (spending plans). Monitor quality and safety of care for Veterans enrolled in VDC. Conduct quarterly reviews and complete required documentation for Personal Care Services. Maintain ongoing communication and coordination with VDC provider(s). Attend and actively participate in monthly VDC provider meetings. Telework: Available.
