Responsibilities Primarily provide outpatient rehabilitation services in an in‑home setting for older adults. Evaluate and treat patients in the most functional environment: the patient’s home. Assess, develop, and deliver physical therapy programs to facilitate rehabilitation. Identify needs and make referrals to other disciplines to address comprehensive patient needs. Implement individualized therapy programs to restore, reinforce, and enhance performance. Instruct, educate, and train patients and caregivers in skills essential for independence and productivity. Direct activities to promote and maintain health. Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel as applicable. Ensure adherence to department and patient treatment schedule, timely clinical and billing documentation, and meetings. Attend and contribute to patient care, staffing conferences, and related meetings. Consult with other rehabilitation team members to select appropriate therapeutic regimens. Adapt program and plan of care according to patient needs. Promote a safe environment and enforce elimination of fire and safety hazards. Order supplies and equipment as necessary. Maintain and report equipment needs for repair. Provide patient‑centered service ensuring highest quality care in a compassionate atmosphere. Screen, examine, and evaluate patients: history, systems review, tests, synthesis, diagnostics, predict improvement, design plan of care. Develop treatment goals and methods in collaboration with patient and caregivers; implement plan and complete documentation. Perform therapeutic techniques: therapeutic exercise, functional training, manual therapy, airway clearance, integumentary repair, electrotherapy, physical agents, mechanical modalities, health and wellness education, chronic disease management, falls assessment, case management, adaptive devices, community mobility assessments, etc. Design, fabricate, fit, or apply adaptive devices and train patients on their use. Select appropriate activities based on physical and mental capacity, intelligence level, and interests. Instruct patient in mobility skills and evaluate progress. Perform routine re‑examination and progress plan adjustments. Incorporate health and wellness programming into treatment plans. Perform home and community mobility assessments and recommend safe mobility. Perform other related duties as required. Qualifications Valid Physical Therapy license in the state(s) of practice or proof of license eligibility. Master’s degree in Physical Therapy or a related field, or Bachelor’s degree in Physical Therapy plus five years of progressively responsible PT experience. For degrees earned outside the U.S., assessment that they are equivalent to a U.S. Physical Therapy degree. Valid driver’s license or reliable, independent transportation to patients’ residences.
