Description: The RN implements professional nursing practice centered on the patient as a whole person. The focus is on creating a caring environment that emphasizes healing and wellness. Nursing practices are based on evidence, current research, policies, procedures, regulatory standards, and professional guidelines. Clinical decision-making incorporates all four phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through helping relationships among nursing staff and across other disciplines. Coordinate, ensure, and provide direct patient care activities on the unit. Responsibilities include nursing rounds, assessments, and treatments. Facilitating admission, transfer, and discharge activities is essential. Provision of patient and family-centered education is a priority. Care coordination and individualized treatment planning are applicable. Assessment of Patient Needs includes performing and documenting comprehensive assessments. Consider the patient’s culture, age, and developmental level. Identify patient risk factors and presenting complaints. Obtain patient health information from family or others as necessary. Collect objective, quantitative, and subjective, qualitative data. Verify initial admission assessments, if obtained by other members of the healthcare team. Develop a problem list, prioritizing patient needs based on input from the inter-professional team as appropriate. Design actions and interventions relevant to patient’s culture and age. Determine desired outcomes in coordination with the patient and family in accordance with the medical plan of care. Fully implement patient care therapies and procedures within scope of practice. Explain procedures, treatments, and processes to patients and families. Initiate actions to alleviate patient stress and discomfort. Recognize, respond to, and communicate changes in patient conditions appropriately. Conduct and document ongoing assessments and responses to procedures and therapies using the nursing process. Set priorities for the care of multiple patients. Ensure resources and coordinate the plan of care. Organize workload for effective utilization of time and energy relative to patient acuity and care processes. Delegate tasks appropriately and oversee team members in performing activities regarding patient care. Monitor the accuracy and completeness of delegated patient care provided by other team members. Identify and address educational needs of patients related to prioritized problems according to their level of comprehension. Facilitate the patient discharge process by executing discharge orders in a timely manner. Provide input into patient/nurse assignments considering available staffing, unit activities, and patient acuity. Demonstrate active participation in unit-based activities in support of organizational values and priorities. Promote a positive group dynamic and healing environment through words and actions. Assure nursing excellence in accordance with accepted standards, evidence-based practice, current research, and a caring philosophy. Actively participate in quality improvement activities and core measures. Use a holistic perspective regarding patient care.
